So I found an Old Pokedex in a box, and I'm wondering what it's used for. My theory is that it's used as a forme-changer for Rotom, to get the Pokedex Rotom, but I want to be 100% sure. Disclaimer: I don't actually own a Rotom

Yes, you are correct.

You give it to a Rotom as a held item and it changes its form into Rotom Pokédex.
