Here is an example of the filled-out form:
Finnegan! I want a Gijinka, please!
Username: Examplurr (obviously your username goes here)
Pokemon Sprite: Charizard (if you want a specific sprite you can list as follows) FireRed Charizard
Trainer Sprite: Pike Queen Lucy (if you have a certain sprite in mind, just list it, if not I will pick one that I think will work or that matches your reference photo)
Reference Photo: None (okay for this just leave a link to a Gijinka picture you might've decided you liked or that you made, this one is completely optional)
Specific Features: Wings and tail (here you put anything you definitely want me to include and I'll make sure that it is included)
Shiny?: Yes (pretty straight forward, just say if the Pokemon you want is shiny or not)
Personality: Attitude, tough, cool (this is optional, but if you do not specify a trainer sprite or provide a reference photo, please fill this out so I know what kind of trainer attitude I should look for, like tough, playful, shy, etc)
Anything Extra: Make the clothes match the red on Charizard's wings (here you list any little detail you want added that would otherwise probably not happen)
Magical Password: Hidden in the rules (as proof of reading the rules, I ask that you supply the password here. The password changes with each request list so that people cannot just look at old requests for the password and I don't have to worry about people editing it out)

Reference Photo: This must be in a spoiler, I didn't put it in one because this form is already in one and you can't double spoiler. Please do not use a gif! I only used one because I like them and I already did this sprite. Gifs will make it hard to look at for reference.
Specific Features: Hair and attitude, original/usual outfit (not battle outfits) Here you put anything you absolutely want me to include, usually something central to the character that makes it clear who it is
Anything Extra: I can tip, I am okay with free-handed parts This is just where you list any sort of message to me about the sprite, such as tipping or things not mentioned in the form otherwise
Magical Password: Hidden You must read the rules to find it

Pokemon/Character: Mew (You can pick a Pokemon or a character from somewhere else. I will always do Pokemon, but I reserve the right to decline a character.)
Tongue Color: Pink (This is the color the lickie's tongue will be. If you want a 3d feel, request to have the tongue "press" the screen in the Anything Extra section.)
Background Color: Light Green (This is the color behind the lickie.)
Shiny?: Yes (Here you say yes or no if the Pokemon shiny or not.)
Anything Extra: Tail in the background (Here you ask for anything extra, such as a specific feature or if you want the tongue to appear to "press" on the glass or screen.)

Pokemon: Charizard (Here you put the Pokemon you want in the Pokewalker.)
Color or Grayscale: Color (Here you say whether you want the Pokemon to be in color or in Grayscale, like in an actual Pokewalker.)
Pokeball Color: Original Pokeball (This is the color or type of Pokeball you want. If given a picture or description, I can do a a custom Pokeball.)
Shiny?: No (Here you say yes or no if the Pokemon shiny or not. If you pick Grayscale, this does not apply unless the shiny colors are extremely different, enough that the shades of gray would change.)
Pokemon Animation: Bobbing. (Here you can pick if the Pokemon is animated, bobbing- moving up and down, stationary- not moving, or walking- moving back and forth across the screen.)
Anything Extra: Nothing. (Here you ask for anything extra, such as a specific feature or color scheme, etc.)

1. Tipping: While I do not have a set price, I do ask that if you are really happy with your sprite that you send a tip, especially if you have chosen a difficult or complicated sprite. I enjoy making these sprites for you guys, but they do take a lot of time and effort.(there are no bans or warnings (PW: Sans)

2. Requesting while slots are full: I have a limit for a reason. I am very busy usually because I'm in college and I also work, along with having other responsibilities to keep up with. Please do not try to request when the slots are full. (requesting while slots are full gives 1 warning for each request)
3. Requesting multiple Sprites at a time: Due to the requests limit being so low, I ask that you request only one Sprite at a time in a category. You can have more than one request in multiple categories, such as one lickie and one Gijinka, but not two lickies or two Gijinkas. If I increase or remove the limit later, you may ask for more than one, but for now one at a time. (Requesting more than one sprite in a category at a time will result in a warning) 4. Rushing or complaining to me: As I've said, I am busy. I will get these done as quickly and as nicely as I can, but I do not have all the time in the world. I will be more active at my dad's house than my mom's. Most of my free-time is during the night. Also, if you do not like an aspect of your Sprite, tell me in a polite way- don't whine about it. I'll be more than happy to fix it for you. (rushing, nagging, complaining, etc will result in a warning)
5. Incorrect form: Please use the form I gave. You can personalize your answers if you so wish, but I won't accept any requests not in that form, such as simply saying "I want an Espurr Ginjinka." (requesting without the form will result in a warning)
6. Rudeness: I do not tolerate rude behavior in my shop towards me or other customers. If you're being rude you will get a warning. If you continue to behave rudely or refuse to apologize you will be banned. You may give constructive criticism, but popping in, saying "These suck," and leaving is not okay and won't be tolerated.
7. Punishment system: Breaking a rule will give one warning which will be taken away after a week. Accumulate three warnings and you will earn one ban. Bans last for one week unless I specify otherwise and they remain listed after wearing off. If you get three bans or post while banned, you will earn a perma-ban.
8. Posting while banned: If you post while banned for any reason other than to ask me a serious question, you will be perma-banned. (an example of a serious question would be something like "How many days are left on my ban?" Posting spam by ordering despite the ban or simply bothering other customers is not acceptable.)
9. Giving Credit: Please see the Credit Policies below. If you are caught claiming my work as your own you will be instantly perma-banned and reported for art theft. If you are caught using my work without providing credit, you'll be asked to do it. Repeated refusal to give credit will result in a perma-ban and report for art theft. Please report any suspected art theft of my work (including using the PokéWalker bases except the original) to me before reporting it elsewhere. There may just be a misunderstanding.
10. Friending Me or Subscribing to the Thread: While you have an order active in my shop, I ask that you add me to your Friendlist so that I can PalPad you and find you more easily (this especially goes for users who allow PalPad from friends only). As soon as your sprite is done you may unfriend me, or keep me, either is fine. You can also subscribe to the thread until your order is done if you'd prefer. This way, you can keep an eye on order progress and I don't have to hunt you down. I give one notification of an order's completion via PalPad.

Do not claim my sprite(s) as your own work.
Please make your Sprite a link to my shop as a way of giving credit.
If you planned to use the Sprite as a link to something of your own, list underneath it that it was made by me. You may use my name or username, Finnegan or ComicSans.
If the Sprite will be used for something like an RP where it will be posted a lot, either use the code with my shop link or give credit the first time you used it (such as a sign-up or first post) and anytime someone asks about it.
If you use the Sprite as an avatar, your About Me (on any site you use my Sprite on) must give credit by giving a link to this shop.
You may order the Sprites, but I make them, and I reserve the right to use them as I please without asking you permission. I'm not so cruel as to use it where you do, but they are mine as well as yours and I can use them.
*Disclaimer: original PokéWalker sprite comes from here. I made the other types of Pokeballs, and I added all the Pokémon that are shown in this shop. To use them you need permission.
