how do i equip items on my pokemon

Go to your Item Bag, next to your items you will find "Give Item" and "Use" links (depending on which item you are looking at). They do exactly that. After that you will be redirected to a window showing your Boxes and Pokemon. Pick the Pokemon and confirm with the button at the bottom.
"Use" will let you use the item on the Pokemon. This is done especially with evolution items like Fire Stones.
"Give Item" will give your Pokemon an Item to hold. This is needed for Everstones or temporary Formechange items.
