What is it exactly?

When you type a smiley directly (or a smiley followed by a whitespace) into someone's Pal Pad (or in the Notifications), that BB Code doesn't get converted into a smiley.

What is the solution?

Maybe Riako should re-build the conversion system (or possibly, use the NBBC library). :)
