I didn't realize this was a bug until one of my friends told me. I was going through my released pokemon and recovered a Simipour (Waiter). Realizing I had some more Waiter clothes I didn't need to recover it so I put it up on the auction house..

But then I was told the waiter clothes are a forme change item and should be removable from the pokemon. The Simipour (Waiter) is not holding waiter clothes and doesn't seem to have a way to revert to original Simipour (not a waiter).

Edited to add:
Seems to be an issue with all forme change holding mons released, I just recovered a Frost Rotom and have the same issue.

Have you tried giving it a Waiter clothes and removing it again?

That does revert the mon back to normal (Just tested with frost rotom) but that still doesn't explain the missing items upon release/recovery..
